＃白色組可分別供個人或家庭參加，家庭組每組最多四人，隊員最少須包括一名14歲以下兒童

及一名18歲或以上人士 White Class is opened for individual or family to join. Each family group could

consist of a maximum of  4 people, in which at least 1 participant below the age of 14 and 1 participant with the age of 18 or above should be included

 

報名費           ：個人組 Individual    (18歲或以下 Aged 18 or below)          － 每場賽事HK$25 /event

Entry Fee                                            (18歲以上 Aged above18)                  － 每場賽事HK$30 /event

家庭組 Family         (每個家庭 Each family)                       － 每場賽事HK$45 /event

(年齡以2005年1月1日當日計算 The age is counted on the 1st Jan 2005)

 

獎項               ：設每場每組冠、亞、季軍獎

Prizes                   The first 3 places in each class of each event will be awarded

 

報名               ：填妥報名表連同劃線支票(抬頭請寫”香港野外定向總會”)寄往香港銅鑼灣掃桿埔大

Application          球場徑1號體育大樓1014室，支票背面請寫上參加者姓名

Mail the completed entry form with a crossed cheque payable to "Orienteering Association of  Hong Kong" to Room 1014, Sports House, 1 Stadium Path, So Kon Po, Causeway Bay, Hong Kong. Please write down the name of the participant at the back of the cheque

備註 Remarks:

1.                  本賽事將採用電子打孔系統，電子控制咭由大會提供

Electronic punching system will be used and E-card shall be provided by the Association

2.                  賽事資料於賽事舉行前一星期上載於總會網站www.oahk.org.hk，各參加者必須自行瀏覽Event information shall be uploaded on OAHK website: www.oahk.org.hk one week before each event. Participants must visit on their own

3.                  大會不接受傳真報名及期票  Application by fax and post dated cheque will not be accepted

4.                  大會已購買公眾責任保險，參加者/家長可考慮自行購買其他保險Public Liability Insurance is covered by the Association. Participants/Parents are recommended to apply other insurance

5.                  你所提供的資料只用於本會與合辦機構的康體活動報名事宜及活動宣傳之用。在遞交申請表後，如欲更改或查詢你的個人資料，可與本會職員聯絡The information provided by you will only be used for the enrollment and promotion of recreation and sports activities organized by our Association and co-organizing parties. For correction of or access to personal data after submission of this form, please contact the staff of our Association

6.                  大會保留最後修訂之權利Organizer reserves the final right for any amendment
