資格 Requirement ： •6至13歲兒童, 不限性別 及

Children aged 6 to 13, no sex limit    AND

•就讀九龍區學校

丙　組	C Grade	MC	WC	12 – 13歲	Aged 12 to 13	1998 – 1999
丁　組	D Grade	MD	WD	10 – 11歲	Aged 10 to 11	2000 – 2001
戊　組	E Grade	ME	WE	8 – 9歲	Aged 8 to 9	2002 – 2003
己　組	F Grade	MF	WF	6 – 7歲	Aged 6 to 7	2004 – 2005

報名費 Entry Fee ： 每位港幣 $20   HK$20 per person

獎項Prize           : 每組前6名可獲頒發獎牌及證書

Medals and Certificates will be awarded to the best 6 places in each class

出席證書 ： 賽員可於報名表內申請出席證書，每張港幣$20元正

Certificate of Attendance：          Participants can apply the Certificate of Attendance and the fee is HKD$20

報名 Application     ： 參加者請填妥報名表格，附上劃線支票(抬頭請寫”香港野外定向總會”)
寄回香港銅鑼灣掃桿埔大球場徑1號奧運大樓1014室，
支票背面請寫上參加者姓名及賽事名稱

Mail the completed entry form with a crossed cheque made payable to the
 “Orienteering Association of Hong Kong” to Room 1014, Olympic House, 1 Stadium Path,
So Kon Po, Causeway Bay, Hong Kong. Please write down the name of the participant and

備註Remarks：    1. 賽事資料於賽事舉行前一星期上載於總會網站www.oahk.org.hk，
  各參加者必須自行瀏覽

Event information shall be uploaded on OAHK website: www.oahk.org.hk one week before
 the event. Participants must visit on their own.

2. 大會已購買公眾責任保險，參加者/家長可考慮自行購買其他保險

Public Liability Insurance is covered by the Association. Participants/Parents are
recommended to apply other insurance

3. 大會保留最後修訂之權利

Organizer reserves the final right for any amendment
